Many wonder what it truly takes for a health program to create enduring change, extending far beyond its initial implementation. The answer lies in sustainability—the ability to ensure that services not only make a difference today but continue to improve lives for generations. This involves more than just service provision; it’s about embedding these services to achieve lasting health impact.

Here are four key strategies ECEWS employs to achieve this, offering a blueprint for pioneering health initiatives:

1. Integrating Services with the Government Systems

Imagine trying to build a house without a solid foundation; it wouldn’t stand for long. The same goes for health programs. A cornerstone of long-term success is embedding health services directly into a country’s existing national healthcare framework. This approach isn’t just about making things easier; it’s about making them endure. By working hand-in-hand with government health systems, initiatives can ensure that positive health outcomes aren’t just temporary gains. This helps to build local capacity, reduce reliance on external support, and empower local health managers and communities to take ownership and adapt services independently.

On the ECEWS ACE-5 Project, our approach has been to co-locate ART centres with the larger health system. Rather than establishing isolated clinics, pharmacies, or laboratories with donor-funded personnel and equipment, ECEWS advocates the integration of the ART clinics, pharmacies, laboratories, and other service areas into the existing health facility infrastructure. This embedding transforms the program into an integral part of the broader healthcare system, reducing dependence on donor funding, while communities and local structures become active participants in their own health journey.

2. Leveraging Technology for Efficiency and Scale

Technology is a transformative force in modern healthcare. Investing in digital innovations can significantly streamline operations and facilitate more informed decision-making. Instead of burdening healthcare workers with extensive manual documentation across multiple platforms, smart automation tools can efficiently transfer data between different health systems. This drastically increases accuracy and frees up staff. Such tools can automate a significant portion of daily data entry, allowing dedicated health workers to concentrate on strategic, people-centered activities.

Our Digital Records Assistant exemplifies this principle. We advocate and have developed tools for a “single data window,” where healthcare workers only need to document information into service forms. The subsequent processes of aggregation, reporting, and data visualization are then automated.

3. Engaging Hearts and Minds using Local Expertise

Sustainability isn’t just about systems and technology; it’s deeply rooted in the people. Successful initiatives actively strengthen support for individuals, families, and communities by offering education, empowerment, and connections to crucial services. Our Enhanced Expert Support Initiative (E2SI) connects medical experts from tertiary institutions with primary healthcare workers to improve their capacity to deliver higher-quality services. This is done through scheduled or unscheduled virtual consultations and in-person mentoring, discussions, and practical demonstrations. By leveraging already employed medical experts, the initiative also ensures cost-efficiency and inherent sustainability. Bridging the gap between experts and frontline healthcare workers ensures that the vast majority of clients, who receive care at local facilities, benefit from specialized guidance and mentorship.

4. Innovative Service Delivery and Simplification

A hallmark of sustainable programs is the continuous effort to simplify and optimize service delivery tools and processes. This includes consolidating meetings for maximum efficiency, regularly reviewing and updating program tools, and eliminating duplicate efforts. Promoting the sharing of best practices across different sites ensures that successful interventions are effectively scaled and sustained.

When all these strategies come together, they create a powerful blueprint for achieving lasting health impact. By embedding these principles into every aspect of their work, ECEWS is not only achieving great results today but also building robust systems, nurturing essential skills, and fostering relationships that will continue to benefit Nigeria’s health for generations to come.
